Initial Consultation
This initial consultation via phone or video call is the starting point on your journey with us. One of our experts will connect in order to obtain more information regarding your or your family member’s condition and help design a comprehensive and personalized care plan.
A consultation is required prior to any in-home assessment to make sure that the care plan best suits the patient and is offered free of charge.

Comprehensive in-Home Assessment
Our experts will come to your loved one’s home in Westchester or Rockland counties in NY to provide guidance and advice to help transform the space into one which is friendly to people with cognitive difficulties.
Our expert will provide two one hour sessions. The first session will be an assessment to plan the changes discussed in the initial consultation. The expert will then return for a second one hour session with personalized materials.

Monthly Check-In Plan
This is a monthly subscription-based plan where patients and families can continue to receive support from our team with unlimited online messaging and a 1 hour monthly in home session to update, replace and modify the changes we made during the in-home assessment

Assessment of Additional Settings
Caring Minds will provide a comprehensive assessment of a secondary environment in addition to the patient’s primary residence if they split their time between different locations, such as a second home or child’s home. This service and only be purchased if you have previously purchased a comprehensive in-home assessment. This service can also be purchased multiple times if more than two environments are required.

Cognitive/Language Therapy
A licensed speech-language pathologist will meet with your loved one to provide dedicated cognitive or language therapy sessions. This would include a formal speech and language evaluation, identification of deficits, and designing a treatment plan based upon these findings.

Family Training and Education Sessions
One of our experts will meet with family members and friends to explain the major features of your loved one’s cognitive disorder, answer questions and provide suggestions on the best way you can support them.
